General Practitioner Forth Valley Area - complaint was from Mr C about the post surgical care and treatment he received from two General Practitioners (GP 1 and GP 2) at his local practice (the Practice) in August 2004 following his vasectomy.

Fife Council - complaint was from the leader of a youth group (Mr C) who claimed that Fife Council had been guilty of maladministration in relation to a youth group’s proposals to develop a skateboard facility in a Fife village. Mr C alleged that the Council had failed to give proper information from the outset with regard to the planning position and that, subsequent to the group’s appeal to Scottish Ministers, had not allowed the group to implement their planning consent and had colluded with others to subvert the democratic process with regard to a ballot taken in the community.

North Lanarkshire Council - complaint on behalf of householders (referred to in this report as Mr and Mrs C) against North Lanarkshire Council, regarding the Council’s handling of the development of a house on a plot adjoining their own.

Perth and Kinross Council - complaint was from Mr C against Perth and Kinross Council (the Council) concerning traffic management changes which were introduced in Coupar Angus about three years ago. He was aggrieved at the way in which the Council handled his representations.

North Lanarkshire Council - complaint was from an individual referred to in this report as Mrs C who complained that North Lanarkshire Council (the Council) had refused to allow her to retain a closed circuit television camera she had installed at her home.

East Dunbartonshire Council - complaint was from a man (referred to in this report as Mr C) against East Dunbartonshire Council (the Council) concerning his request for the removal of trees, owned by the Council and overhanging his property.

Perth and Kinross Council - complaint was made online from a resident in the Perth and Kinross Council area (Mr C) who complained about the arrangements made by the Council for publishing their Draft Eastern Local Plan.

Falkirk Council - complaint was against Falkirk Council from the owners of a house (Mr and Mrs C) who were concerned about aspects of their neighbours' rear extension. At that stage, the Council's complaints procedures had not been completed. After considering the Council's response which they received on 26 September 2005, the complainants asked this office to pursue matters on their behalf.

Forth Valley NHS Board - complaint was from a woman (referred to in this report as Mrs C)who complained that Forth Valley NHS Board (the Board) failed to provide her with adequate clinical care and treatment at the Stirling Royal Infirmary (the SRI) during her admission for the birth of her third child on 4 April 2005.

Tayside NHS Board - complaint was from a woman (referred to in this report as Ms C) about the decision of one of the conveners used by Tayside NHS Board's Primary Care Division not to convene an Independent Review Panel to examine her complaint.
